OPC Server for AC 800M not running, getting OPC Bad Quality error
When I click OPC Server for AC 800M app it does not come up means not running.
While I checked AC800MC_OPCService is on Manual and running.
Because of this, getting OPC bad quality error for HMI Tags/Description etc. How to fix?

The OPC Server log file will likely contain some relevant information, please collect it from C:\ABB Industrial IT Data\Control IT Data\OPC Server for AC 800M\LogFiles
In fact make a ZIP of all log files in that folder and attach to your thread.

AC 800M Service Provider DA (from AC 800M Service Group>OpcDA_Connector, Service) is showing "Initialize", Options to to RUN/STOP it are invisible under Command. Same for Event collector. Error screenshots attached. How to fix?
Also attached latest OPC log file (ABB forum does not support uploading Text and Compressed files).

I believe you realise after studying the log file that the AC 800M OPC server is having major issues connecting your controllers (especially 172.16.80.191 repeatedly reported disconnected/reconnected).
I have not seen this behavior before - the AC 800M OPC server was rebuilt for 6.1, so we are dealing with a first release of something new.
The OPC DA Connector and Event Collectors are expected to not reach Service State; these services will wait until the OPC server reports "OPC Running" on the GetStatus() call via OPC. You can even notice this abnormal state with OPC DA test clients like our own advdsopcclient.exe.
You need to resolve what is bugging the OPC server.
I see traces of four controllers being connected; two soft controllers and two hardware controllers.
I suggest removing all controllers from the configuration and add them back, one at a time.
Ultimately, file a support case with your regional ABB support center to obtain professional support.

Did you check if opcenum windows service is running?
Thanks to all who answer especially Stefan. OPC issue is solved by ABB support. This is how:
NOTE: It is 6.1 version, some bugs are fixed in next version update. So we are going to update also.
- Uninstalled and Re-Installed OPC server.
- Restarted Server
- Removed all controllers from AC 800M OPC Server Configuration Panel.
- Add only one Controller (IP of primary).
- Done.
