Yes, if the 1st FC132 status output is at least changing state and quality, there is some recognition of the slave module on the expander bus by the controller.
What is the FEC12 status LED doing? It should be green and solid?
What happens when you connect a transmitter / loop simulator to a channel? For system powered inputs there should be 1-5V between IO power 0V and the channel -ive terminal.
What does the controller module problem report indicate?
What do the +/-15V supply voltages look like?
If it doesn't appear to be a power or installation issue, do you have another FEC12 to try in the slot? The module could always be DOA.
Other more random installation issues that I have come across include
+/-15V supply not wired to every MMU.
FC132 default channel span setting is 0 (unused), which gives 0 and BadQ as an output.
Dipshunts wrong way around for the orientation of the NTAI05.
Initial installation of the FEC12 has pushed the NKTU01 connector out of the MMU.

Verify appropriate FC block ordering in your configuration. The FCs must be configured in ascending block order as follows: FC-146 >> FC-147 >> I/O FCs linked to FC-147.
Another thing to Verify is that the controller switch settings be appropriate. For controllers BRCxxx ensure that the cache memory switch settings (BRCxxx = SW4-6,7, and 8) are configured properly.
FEC12 modules utilizing the MC68H11A... processor (U28) are compatible with only the A.x and B.x firmware revisions. Should you have the "A" vintage processor with the C.x firmware revision installed, it will be necessary to either downgrade the firmware to the B.x revision or upgrade the FECxx module to the latest FEC12 module to support the C.x firmware.
