I have checked our Knowledge Database and it could but a Cable that has become loose or even the Connector Corroded. Please Check all cables for any Corrosion or Poor Contact. Pull re-insert and try a re-Power.

Hi, OIS41, what a beast.. The system hanging at the "Now Activating OIS Software" prompt can also be caused by the OIS disk, DKA300 not being available or corrupted. The OIS boots VMS from the system disk, DKA200 and then the OIS/CVMS application runs off the OIS disk, DKA300. Without DKA300, you can have a functioning VAX station that just doesn't work as an OIS console. You can interrupt the boot and look at the SCSI devices available and you can also do this from a VMS terminal window with "show device". The error logs may also be useful in identifying a disk problem. If DKA300 is available as a device you could try restoring DKA300 from a backup or installation media but generally once the disk starts having reliability issues it is a slippery slope to complete disk failure.
