Need help: MicroFlex E150 servo drive "phase search failure"

I'm relatively new to servo motors and drives thus I have a problem.
I have a MicroFlex E150 drive and an old used Reliance h-4038-m-h servo.
Mint WorkBench completes successfully the feedback test however I'm getting a "phase search failure" (10020) error when I run the "measure the inertia" test. The rotor of the motor revs about half a revolution then stops and the error message comes.
Phases of the motor is described as RST which I assume that means UVW, so I did the wiring but maybe I'm getting the 10020 error because I wired the phases wrong?
Any help would be appreciated.

The Phase search error 10020 is part of the tuning process associated with the feedback device. Due to the logic the "failed tick box" might appear to be during the Inertia test but its actually not. When the drive is first enabled a 'phase search' sequence of movements is used to determine the position of the rotor within the electrical cycle related to the feedback devices angle. In the simplest case, this phase search algorithm applies current to the stator windings to generate a magnetic field in a known orientation. The magnets in the rotor are then allowed to align freely with this field. After allowing a suitable period for motion to settle, the rotor is assumed to be in the same orientation as the field. This information is used to set up commutation. In practice, the phase search algorithm available on drive products implements measures to improve the accuracy of alignment, and to detect if an end stop or limit switch has been hit during the alignment process.
So normally when we see this error you're probably looking at an issue with the feedback device / cabling / EMC / Mounting. To check basic operation you can monitor it, rotate the motor by hand and check the encoder count. If you want more detail after the test fails you can go to Tools > Upload capture data and it will show you the graph it generated during the test which should help show you where the error is.
